Submandibular salivary gland

Site:

Is a large salivary gland (1/2 the size of parotid), wedged between mandible & mylohyoid muscle in the submandibular fossa of the mandible.

Extensions:

(superficial part)

       Superiorly: To mylohyoid line.
      Inferiorly: Overlaps intermediate tendon of digastric.
       In front: To mental foramen.
      Behind: To angle of the mandible, separated from parotid gland by stylomandibular ligament.

Parts

2 parts:

1.      Superficial part: Superficial to mylohyoid muscle.

2.      Deep part: Deep to mylohyoid muscle

ü  Both parts are continuous with each other along the posterior border of mylohyoid muscle.

Relations:

A- Superficial part: three surfaces.

1) Inferolateral surface: Is related to:

¦ Skin, fascia, and platysma.

¦ The submandibular lymph nodes lie on it.

¦ It is crossed by:

1. Cervical branch of facial nerve.        2.  Facial vein

¦ Its posterior end is grooved by the facial artery.

2) Lateral surface: Is related to:

a.       Submandibular fossa of the mandible.

b.      Facial artery between it and the mandible.

c.       Insertion of medial pterygoid muscle.

3) Medial surface: is related to:

¦ Anteriorly:

1)      Anterior belly of digastric.

2)      Myelohyoid muscle, N. & A.

¦ Postetiolv:  Posterior belly of digastric &  Stylohyoid.

¦ In the middle: - Hyoglossus muscle & its lateral relations.

B) Deep part:

¦ It appears as a tongue like process from the medial surface.

¦ It lies between:

§  Mylohyoid (superficially) & hyoglossus (deeply).

§  Lingual nerve (above) & hypoglossal nerve (below) .

¦ Submandibular duct arises from its lateral part.

Submandibular (Wharton's) duct:

¦ beginning :  

arises from the deep part of the gland (5 cm).

¦ Course : 

It runs forwards and upwards between :

1.      Mylohyoid (superficially) & hyoglossus(deeply).

2.      Lingual nerve (above) & hypoglossal nerve (below).

-          At the anterior border of hyoglossus muscle, the lingual nerve crosses the duct twice relations) superficial, then below and finally deep to the duct.

¦ opening :  

opens in the summit of sublingual papilla (on side of the frenulum linguae).

Arterial supply:

Facial artery (glandular & submental branches).

Nerve supply:

- Sensory: Lingual nerve.                          

- Sympathetic: Plexus around the facial artery.

- Parasympathetic: Chorda tympani of facial nerve.


Applied anatomy: (Submandibular gland stone)

1- Its secretion is thick & viscid.                        

2- Its duct is narrower & runs against gravity.

3- The duct opening may be obstructed with food particle.

Submandibular Ganglion

¦ Site:

  • In submandibular region, suspended from the lingual nerve on the outer surface of hyoglossus.

¦ Roots:

  • Sensory root: Lingual nerve.

  •   Parasympathetic root: Chorda tympani N.

  •   Sympathetic root: Plexus around facial A.

¦ Branches:

  •   Parasympathetic, sympathetic & sensory to: submandibular & sublingual glands.
